Make a consent form fillable
The wording stays exactly as approved. Only the blanks become fields.
Consent forms are the worst case for retyping: the wording has usually been approved by someone, whether that is a clinic, a school, a lawyer or an ethics committee, and it must not change by a single word. That rules out rebuilding the form in a form service. Adding fields to the existing PDF leaves the approved text untouched and only makes the blanks typeable, so the document that gets signed is the document that was approved.
How to make a consent form fillable

Open the approved form
Drop in the PDF exactly as it was approved. Nothing in the text is altered.

Turn the blanks into fields
Name, date, relationship and the tick boxes for what is being consented to. Untick anything that should stay a printed line.

Save and circulate
Download it and send it out. The signature can be added with the Sign PDF tool, also on the signer device.
Why the document must not be rebuilt
A consent form that has been retyped is a different document, and whoever approved the original did not approve yours. Adding fields to the original avoids the whole question.

Does the wording change at all?+
No. The existing page is kept byte for byte as a page; the fields are new objects placed over the blanks. Nothing in the approved text is rewritten.
How is it signed?+
Sign PDF lets the signer draw, type or place a saved signature, in their own browser. For a stronger record, flatten the signed form so the answers can no longer be edited.
Can the answers be locked afterwards?+
Yes. Flatten PDF turns the filled-in values into part of the page, which is what you want before filing a signed consent.
Is any of this uploaded?+
No. Consent forms carry health and personal details, so it matters that both building and filling happen in the browser, with no server involved.
