What was sent, who received it, and when they signed for it.
A delivery note travels with the goods. It lists what is in the consignment without the prices, references the order it belongs to, and leaves space for the person receiving it to sign. That signature is the evidence that the goods arrived, which is exactly what you need when a customer says two weeks later that they never came.

Why the signature is the whole document
The list of goods is useful. The signature at the bottom is what settles a dispute about whether the delivery happened, which is the only reason the paper exists.

Should the delivery note show prices?+
Usually not. The person receiving goods at a loading bay does not need to see the commercial terms, and many customers prefer it that way.
Is a delivery note the same as a challan?+
In practice they serve the same purpose: a document listing goods in transit and receipted on arrival. Local rules may add requirements, so check what your trade expects.
How many copies?+
Two at least, so a signed one comes back to you. Three if the transporter keeps one.
